Description:The Immunization Data Quality Self-Assessment (DQS) Tool is a flexible toolbox of methods to evaluate different aspects of the immunization monitoring system at district and health unit levels. The DQS aims to assist countries in diagnosing problems and to provide orientation to improve district monitoring, as highlighted in the Reaching Every District approach. This document is to be used primarily by staff who will adapt the toolbox for a specific area (usually staff at national and regional levels). The adapted tool should then be used by staff collecting and using immunization data at the national, provincial, or district levels.
