This tool enables the completion of an organizational self-assessment of eight sustainability domains: environmental support, funding stability, partnerships, organizational capacity, program evaluation, program adaptation, communication, and strategic planning. It provides tools to assess domains and develop action plans to increase the likelihood of sustainability.
